Confusion about review scope, required information, request status, or what payment does and does not mean.

Payment issue

Payment-page confusion, return-page issue, duplicate payment concern, wrong amount, stale payment session, or reconciliation concern.

Report

Report clarity

Questions about report sections, status wording, source-review boundary, data-as-of date, or exception language.

CVC

Verification-code lookup issue

Difficulty entering or reviewing a CertivaSource verification code, unclear result language, or mismatch between expected and displayed status.

Correction

Correction or update request

A request to review potential outdated, incomplete, or incorrect information in a CertivaSource record or report output.

Feedback handling rule

  • Feedback is for operations, clarity, and correction workflow improvement.
  • Feedback should not be displayed publicly as a testimonial without explicit permission.
  • Feedback should not be treated as a legal dispute, official appeal, or source-authority correction unless a separate process is defined.
  • Correction requests should be routed for review before any public record changes.

v0.1 implementation note

This staging page defines the feedback taxonomy and customer-facing language only. The live intake route, storage location, email routing, abuse prevention, and privacy controls must be configured separately before collecting production feedback.

Do not collect sensitive identity documents, payment card details, or private credentials through an unconfigured feedback page.
